Pathway of Change is a visual showing the connections between the need, strategies, activities, and ultimate outcome or goals expected.

Project Monitoring Plan (generic template)

Monitoring plans are used to specify your goals, the data needed, your data collection methods, and the plan for analyzing your data, including who will be responsible for each task.
